Key:
- Accept? Accepted forms will be loaded into the back-end database.
- Notice. If a form is NOT accepted, email either a standard form or customize your email to the charity.
- Date when form received.
- Charity Federal Employer ID number (EIN)
- Fiscal year for form.
- Verified? Form meets all state automated checks (items completed, no arithmetic errors, valid EIN, etc.).
- Signatures? Form has required electronic or paper signatures.
- Payment. Type of payment received. (Flag if incorrect amount.)
- Log. View correspondence with charity via email or paper.
- View Form. View a PDF image of the 990 or state form.
